ABSTRACT

According to current data, SARS-CoV-2 virus has the ability to cause multi-organ pathology, leading to acute damage of various organs and systems and long-term consequences characterized by polymorphic symptoms. Recently, a high incidence of invasive mycoses, particularly mucormycosis - COVID-M, has been noted among the COVID-19 complications. The predisposing factor for the development of this pathology is diabetes mellitus, immunodeficiency states, and prolonged use of high doses of glucocorticosteroids. Mucormycosis is characterized by severe clinical manifestations and high lethality, and timely diagnostics of this pathology often represents a difficult problem. The aim of this study was to analyze a clinical case of rhino-orbital mucormycosis in convalescent COVID-19 patient. In the study, there was used mucopurulent nasal discharge from the patient previously hospitalized with a severe novel coronavirus infection. Here, we describe the methodology allowing to isolate and identify a pure mold fungus culture from the biomaterial using methods of routine bacteriology and MALDI-ToF mass spectrometry. Direct microscopy examination of nasal cavity discharge revealed branched non-septic hyphae with a characteristic branching angle, allowing to preliminarily diagnose invasive mucormycosis. Growth of mycelial fungus colony was observed by using Sabouraud's medium with potassium tellurite. Microscopy of the pure culture revealed branching mycelium without septa, broad, with irregular thickness, unsegregated hyphae, and sporangia with a typical column specific to mucormycetes. Analysis of the obtained mass spectra allowed to establish the microbial species identity as Lichtheimia corymbifera. The latter along with other members of the order Mucorales, are known to cause mucormycosis. As a result of antifungal treatment (Amphotericin B) and timely surgical intervention, the patient was discharged from the hospital with prominent clinical improvement and no complaints during further outpatient follow-up period. The analysis of this clinical case showed the lack of alertness in some clinical diagnostic laboratories to detect pathogens of invasive mycoses. To avoid errors, while making a diagnosis, attention should be paid not only to detection of fungal spores in clinical material, but also take into account the structure of mycelium underlying major difference between yeast-like fungi, higher and lower molds. The isolation and identification of a pure pathogen culture allows to confidently verify the diagnosis, timely correct the treatment tactics and monitor circulation of mycotic agents to prevent occurrence of mycoses in most vulnerable patients cohorts.

ABSTRACT

Computed tomography has become the main method of early diagnostics of COVID-19 during the pandemic of the novel coronavirus infection. Based on results of computed tomography of the chest it is possible to diagnose viral pneumonia associated with COVIS-19, to quickly assess the lung damage volume and severity of changes, to perform quick routing of patients and to start antiviral treatment. Hence, 2020 was associated with a rapid increase in the number of computed tomography examinations with corresponding changes in the structure of X-ray diagnostics. The aim of the current study was to evaluate the impact of the pandemic of the novel coronavirus infection on the structure of X-ray diagnostics and collective doses from medical exposure in the Russian Federation in 2019-2020. The study was based on the results of analysis of the federal state statistical surveillance forms 3-DOZ and 30 as well as on the data on Covid-19 morbidity in the regions of the Russian Federation. The results of the study indicate that there were no significant changes in the structure of X-ray diagnostics in 2020 compared to 2019 except for the increase in the number of computed tomography examinations. Their contribution to the total number of X-ray examinations has increased to 8,2% in 2020 compared to 4,6% in 2019. Contribution of other imaging modalities to the total number of X-ray examinations has not changed significantly. In 2020 the number of X-ray examinations decreased by 20% in average, varying from 6% for diagnostic nuclear medicine to 42% for interventional examinations. At the same time, the number of computed tomography examinations has rapidly increased by 60%. The structure of collective dose from medical exposure has significantly changed in 2020. Contribution of computed tomography to the collective dose in 2020 accounted for to 74% compared to 57% in 2019. Contribution of other imaging modalities has decreased by the factor of 1,5-2. Contribution of radiography examinations has decreased to 10,5% compared to 19% in 2019. Collective doses have correspondingly decreased by 20-30% for all imaging modalities except for computed tomography. Collective dose from computed tomography has increased by 71% compared to 2019. The number of all X-ray examinations in the Russian Federation has decreased by 12%: from 294 million in 2019 to 258 million in 2020. Collective dose from medical exposure has rapidly increased in 2020 by 30% to 115 thousand man.-Sv compared to 88 thousand man.-Sv in 2019. ABSTRACT

INTRODUCTION: Chronic prostatitis (CP) has long been considered one of the common causes of male infertility. The influence of therapy for CP on spermatogenesis is well studied. However, recently, especially in the era of the new coronavirus infection (COVID-19), much attention has been paid to the effects of cytokines on the pathogenesis of the inflammatory process, and their impact on male fertility. AIM: To study the effect of a complex of natural antimicrobial peptides and cytokines (Superlymph) on ejaculate, MAR test, oxidative stress (OS), and sperm DNA fragmentation (SDF) in men with infertility and a history of CP. In some patients, the level of cytokines before and after treatment was determined. MATERIALS AND METHODS: A total of 583 infertile men with a history of CP, aged 18-75 years, were screened according to the inclusion/ exclusion criteria. In group I, patients received antioxidant stimulation of spermatogenesis (triovit, trental, mexidol) in combination with a complex of natural antimicrobial peptides and cytokines (Superlymph) one suppository 25 IU at night for 20 days for 1-3 weeks and for 6-8 weeks for 2 months. In group II, antioxidant stimulation of spermatogenesis was prescribed along with suppositories for the prevention of CP for 2 months. The evaluated criteria included sperm analysis, MAR test, oxidative stress in spermatozoa (reactive oxygen species (ROS)) and SDF. The levels of seminal plasma cytokines were also measured before and after treatment in 32 patients using the Bio-Plex Pro panel for the simultaneous determination of 17 human cytokines, the Human Cytokine 17-plex Assay. Statistica-10 software was used for statistical analysis (p<0.05). RESULTS: Of 135 eligible patients, results were collected from 125 men (mean age 31.7 +/- 13.4 years). In group I, there was a more pronounced increase in motility by 42.3%, a decrease in the MAR-test level by 64.7%, oxidative stress in spermatozoa by 56.1%, and SDF by 25.6%, compared to group II (30.4 %, 10.5%, 45.7%, 21.9%), respectively. All these changes were significant with the exception of a decrease in the level of SDF. In group I, there was a predominant increase in the activity of anti-inflammatory cytokines (IL-4, 10, 13), a decrease in the level of pro-inflammatory cytokines ((IL-2, 12). In group II, there was a significant decrease in both pro-inflammatory (IL-2, 6, 8), and anti-inflammatory cytokines (IL-4). The remaining changes were insignificant. Among mild side effects, there were a change in the color of the sclera in 4 (3.2%), anal pain in 3 (2.4%), a decrease in libido in 2 (1.6%) patients. No differences between groups in the frequency of side effects were recorded (p>0.05). CONCLUSION: Antioxidant stimulation of spermatogenesis in infertile men and CP is an effective and safe. The addition of a complex of natural antimicrobial peptides and cytokines (Superlymph) leads to a more profound increase in sperm motility, a decrease in the MAR-test, oxidative stress in sperm cells, which potentiates the effects of therapy. The use of Superlymph is accompanied by a more pronounced positive effect on pathospermia, an increase in anti-inflammatory and a decrease in pro-inflammatory cytokines. In the contrary, in control group, where patients received antioxidant stimulation of spermatogenesis, only a decrease in the level of main cytokines was observed. The effect of a complex of natural antimicrobial peptides and cytokines (Superlymph) on the level of SDF, as well as the relationship with fertility and childbirth rates, requires further study.

ABSTRACT

The aim of the study was to assess the seroprevalence to the SARS-CoV-2 nucleocapsid in the population of the Nizhny Novgorod region against the background of COVID-19. Materials and methods. The work was carried out according to a unified methodology developed by Rospotrebnadzor with the participation of the St. Pasteur. The selection of volunteers for the study was carried out by the method of questioning and randomization. The exclusion criterion was active COVID-19 infection at the time of the survey. A total of 2829 volunteers were surveyed, divided into seven age groups. Venous blood samples were taken from all volunteers from EDTA vacutainers. In the blood plasma, the content of specific IgG to the SARS-CoV-2 nucleocapsid was determined by the enzyme immunoassay. Results. The population level of immunity to SARS-CoV-2 among the surveyed volunteers was 8,4% (95% CI 7,4–9,5). The largest proportion of seroprevalent persons was found among children aged 1–6 years — 15,9% (95% CI 8,2–26,7), the smallest in the group of volunteers aged 18–29 years — 6,0% (95% CI 3,8–8,9). During 3-stage serological monitoring, the proportion of seroprevalent volunteers increased to 22,5% (95% CI 20,3–24,9)%, or almost 2 times from the initial level (p<0,05). of the population of the Semenovsky district (22,7% (95% CI 10,9–41,8)), the smallest — in the city of Dzerzhinsk (2,4% (95% CI 0,5–7,1)%) Among convalescents after COVID-19, specific antibodies were detected in 47,2% (95% CI 30,5–69,6), which is 5,6 times higher than the average for the population. Among those who had verified contact with patients, specific IgG were detected in 17,6% (95% CI 12,9–23,5), which is 2,1 times higher than the average for the population. Among asymptomatic persons who had a positive PCR result, 53,3% (95% CI 30,5–86,6) were seropositive, which is 6,3 times higher than the average for the population. Of 225 seropositive people, 188 (88,4% (95% CI 76,6–101,6)) had the disease asymptomatic. Conclusion. The relatively low proportion of seroprevalent persons among the population may indicate a significant risk of further development of the epidemic process caused by COVID-19 in the territory of the Nizhny Novgorod region. © 2021,HIV Infection and Immunosuppressive Disorders. ABSTRACT

The Bioethics Committee of Belarus and the local ethics committee of the St. Petersburg Pasteur Institute approved the study. Selection of participants was carried out using a questionnaire method and online technology (internet, cloud server). Volunteers were randomized into seven age groups (years of age): 1–17;18–29;30–39;40–49;50–59;60–69;and 70+. Regional randomization ensured proportional representation of volunteers from each region, and no more than 30 people were included from one enterprise. In accordance with manufacturer instructions, blood plasma samples were analyzed for: IgG antibodies (Abs) to the SARS-CoV-2 nucleocapsid (Nc) using a quantitative ELISA test system;and IgG Abs to the receptor binding domain (RBD) of the SARS-CoV-2 S (spike) surface glycoprotein using a qualitative ELISA test system. Statistical processing was carried out using Excel 2010 and other software. Statistical differences were designated as significant when p < 0.05, unless otherwise indicated. Results. The level of seroprevalence, in terms of Abs to Nc among the Belarusian population, was 38.4% (95% CI 37.6–45.4). The highest Ab levels were found among individuals in older age groups (50-70+ years old). The lowest were found in children 1–17 years old and in young people 18–39 years old The distribution of seroprevalence across Belarusian regions was relatively homogeneous, with the exception of the Minsk Region, where a statistically significant decrease in the indicator was noted. In terms of profession, the largest share of seropositive individuals was found among transportation workers;the smallest was found in business. The moderate COVID-19 incidence has not led to a dramatic increase in the number of contacts. The base reproduction number (R0) was 1.3. In the Republic of Belarus, there was a moderate level of asymptomatic COVID-19 among seropositive individuals (45.3% [95% CI 44.0–46.7]). This form of infection was observed most often among children aged 1–17 years old (65.0% [95% CI 61.3–68.6]). In parallel with seroprevalence assessment, SARS-CoV-2 vaccination was carried out. We used two vaccines: Gam-COVID-Vac (also known as Sputnik V, developed by Gamaleya National Center for Epidemiology and Microbiology, Russia);and BBIBP-CorV (Sinopharm, PRC). Vaccination against SARS-CoV-2 was accompanied by an increase in the level of anti-RBD Abs (95% [95% CI 94.7–96.7]). Taking into account the vaccination of a subset of the population with BBIBP-CorV, the overall herd immunity, inferred from the analyzed indicators (presence of anti-Nc or anti-RBD Abs), was 47.1% (95% CI 46.3–48.0). Conclusion. COVID-19 in Belarus was characterized by a moderately pronounced course of the epidemic process. The threshold level of herd immunity to SARS-CoV-2 has not yet been reached, as a result of which the conditions for progression of the epidemic remain.

Relevance. At the end of 2019, the world was confronted with a novel coronavirus (SARS-CoV-2), In January 2020, WHO declared an epidemic related to SARS-CoV-2, a health emergency of international importance, and in March characterized the spread of the world. diseases like a pandemic. Purpose of the study. Conduct a comparative analysis of the seroprevalence of the population of the Sverdlovsk and Chelyabinsk regions – the largest administrative territorial entities of the Middle and Southern Urals – against the backdrop of the COVID-19 pandemic. Materials and methods. The work was carried out under the program of the first stage of assessing the seroprevalence of the population of the Russian Federation according to a unified methodology developed by Rospotrebnadzor of the Russian Federation with the participation of the Saint Petersburg Pasteur Institute. The selection of participants was carried out by the method of questioning and randomization. The presence of antibodies to the SARS-CoV-2 nucleocapsid was determined in blood serum by the enzyme immunoassay. Results. Comparative analysis of the results showed that the incidence rate of COVID-19 in the Chelyabinsk region was consistently lower, and the seroprevalence at the first stage of the study was statistically significantly higher (p <0/05) than in the Sverdlovsk region. It was found that the incidence in both regions had a direct correlation with population density (r = 0.59;p <0.05). There were no differences between the compared areas in terms of seroprevalence among convalescents, people who had contact with COVID-19 patients, and asymptomatic volunteers with a positive PCR test. When analyzing seroprevalent volunteers in both regions, it was shown that the number of asymptomatic individuals varied within 94.4 ± 1.2% – 95.0 ± 0.95%. These data indicate that the majority of volunteers tolerated COVID-19 asymptomatically. Conclusions. A comparative study showed a statistically significant predominance of seroprevalence among the population of the Chelyabinsk region. It was found that an increase in seroprevalence at the population level was accompanied by a decrease in morbidity. Shown is a direct relationship between population density and the incidence rate. More than 90% of seropositive individuals in the compared areas showed asymptomatic course of coronavirus infection. © 2021, Numikom. All rights reserved.

ABSTRACT

Novel coronavirus infection 2019 (COVID-19), which is caused by SARS-CoV-2 (severe acute respiratory syndrome related coronavirus-2), leads to a growing concern for public healthcare worldwide. Question about whether SARS-CoV-2 is able to penetrate into testicular tissue and/or seminal fluid remains unanswered. Fertility specialists also encounter many issues regarding prevention and treatment of infertility in patients with a history of COVID-19. There is an increasing number of publications which demonstrate anti-inflammatory, antiviral and antioxidant properties of so-called adaptogenic plants. This was the reason to analyze published articles on the effects of antioxidants and adaptogens on reproductive function in males with a history of COVID-19. © 2021 ABC-press Publishing House. ABSTRACT

Purpose of the study. Determination of the level and structure of population immunity to SARS-CoV-2 among the population of the Murmansk region against the background of the incidence of COVID-19. Materials and methods. The study was carried out according to a unified method for determining the seroprevalence of the population, developed by Rospotrebnadzor with the participation of the St. Petersburg Pasteur Institute. A total of 3117 volunteers were examined, distributed into 7 age groups. The content of antibodies to SARS-CoV-2 was determined by ELISA using a set of reagents for analyzing human serum or plasma for the presence of specific immunoglobulins of class G to the nucleocapsid of the SARS-CoV-2 virus produced by the FBUN GNCPMiB Rospotrebnadzor (Obolensk) in accordance with the instructions for application. Results. The level of population immunity in the Murmansk region as a whole was 31,2%. The highest seroprevalence was found among children aged 1–6 and 7–13 years (35.6% and 44,1%, respectively), the lowest among the elderly population aged 60–69, as well as 70 and more years (20,4% and 20,9%, respectively). In the cities of the Murmansk region. the herd level of immunity varied from 19,6% (Kola city) to 46,1% (Kandalaksha city). It was not possible to find a reliable relationship between the incidence of the population of the cities of the Murmansk region and seroprevalence. The highest level of seroprevalence was noted among office workers (38,0%), the lowest among transport workers (19,5%). In the presence of contacts with patients with COVID-19, seropositivity increased 1,4 times compared to the average population value. The level of specific humoral immunity in convalescents after COVID-19 is 64,1%, which is 2,2 times higher than the average for half-day. The proportion of asymptomatic forms among seroprevalence volunteers was 89%. © 2021. All Rights Reserved.

ABSTRACT

The novel coronavirus (SARS-CoV-2) pandemic, dubbed COVID-19, has become one of the most serious challenges for human populations in the vast majority of countries worldwide. Rapid spreading and increased mortality related to it required new approaches to manage epidemic processes on a global scale. One of such approaches was based on analyzing SARS-CoV-2 seroprevalence associated with COVID-19. Our aim was to summarize the results on assessing seroprevalence to the SARS-CoV-2 nucleocapsid antigen (Nc) in residents from 26 regions of the Russian Federation, carried out during the first wave of the COVID-19 epidemic. Materials and methods. Seroprevalence distribution was examined in 26 model regions of the Russian Federation according to the unified method developed by the Rospotrebnadzor with the participation of the Federal State Institution Saint Petersburg Pasteur Research Institute of Epidemiology and Microbiology. Such approach implied formation of a group of volunteer subjects in model geographic region who were tested by ELISA for anti-Nc serum antibody level in peripheral blood. Analyzed primary data obtained in separate regions were either accepted for publication or released. Results. The current paper finalizes the data obtained in all 26 regions of the Russian Federation. The total SARS-CoV-2 seroprevalence was 19.5 (10.0-25.6)% with the maximum and minimum value found in the Kaliningrad Region and the Republic of Crimea, respectively (50.2% vs. 4.3%). A pattern of age-related seroprevalence distribution indicates insignificant predominance of seroprevalence among subjects of 1-17 years old: 22.1 (13.1-31.8)%. Among COVID-19 convalescents positive for SARS-CoV Nc antibodies it reached 60.0 (40.0-73.3)%. The number of contact persons comprised 6285 subjects or 8.5% of total volunteer cohort, with the level of seroprevalence reaching up to 25.3 (17.95-35.8)%. A direct correlation was revealed between levels of seroprevalence in convalescent and contact volunteers. In addition, the reproductive number for SARS-CoV was calculated comprising 5.8 (4.3-8.5) suggesting that one convalescent subject can infect at least 4 healthy individuals. A high level of asymptomatic forms of COVID-19 among seropositive subjects was confirmed empirically comprising up to 93.6 (87.1-94.9)%. Conclusion. A single cross-sectional study performed during 2020 June-August timeframe allowed to assess pattern of sex- and age-related COVID-19 seroprevalence for general population in 26 Russian Federation regions. The data obtained may serve as a basis for the longitudinal cohort investigation with serial subject sampling. The timing and duration of study will be determined by dynamics of ongoing COVID-19 epidemic.

ABSTRACT

The review analyses the information on the epidemiological situation on COVID-19 around the world. Presented is the assessment of the major epidemiological parameters (basic reproductive rate, lethality, incubation period, and serial interval). Demonstrated is the share of severe cases among different age groups and the general structure of incidence by the severity of clinical course. The paper provides the classification of response models internationally depending upon the complex of measures undertaken at different stages of epidemic process. Reviewed are the key response actions to control the COVID-19 transmission in different countries of the world and the Russian Federation and given is the assessment of their effectiveness. © 2020 Russian Research Anti-Plague Institute.
